The Jefferson Parish Public School System will continue the following procedure for parents requesting medication to be administered to their child during school hours:

1. Obtain the following forms from your child's school: School Medication Order (S.S.35) and Parent/Guardian Written Consent for Medication Administration (S.S.36).

2. Have your physician sign and complete the School Medication Order Form (S.S.35). The order must agree with the pharmacy label on your child's medication.

3. Take the above completed forms, along WITH MEDICATION in its bottle/container, to either the East Bank or West Bank Medication Assessment Office for review/approval by one of our school nurses (Addresses are given at the end of this letter.)

4. After the forms and medication have been reviewed and approved by the nurse, please take them to your child's school.

Please Note:

1. Emergency medication (Glucagon, Epi-Pen, Mist Machine, etc.) must be approved by the nurse at your school. Please call 361-3823 for an appointment.

2. If your child's medication is to be given only when needed (e.g. Asthma Inhaler), the physician's order must include the number of hours between doses.

3. All medication containers, even over-the-counter medications are to be labeled by a licensed pharmacist or doctor. Sample medication received from your doctor must be properly labeled by the doctor.

4. Please ask your pharmacist to provide separate medicine containers for school and home use.

5. If you plan. to send your child to one of our School System's Summer Programs in 2000-2001 school year, please ask your doctor to date the order from 8/00 to 8/01.

6. Medication must be delivered to the school by an adult.

The above procedure will enable our school personnel to better provide for your child's health needs.

#1 The above medication assessment sites will open for the 2000-2001 school year on August 21St.

#2 If nurses are available in early August to approve medication. before school begins, this information will be available by recordings at the two medication assessment office telephone numbers and at 361-3823. Also, information will be sent to the JPPSS TV station (Cox Cable channel 8).
